Answer to #2 (something I had never heard of): "chopped and screwed" (from the inventor, DJ Screw)
DJ Screw died at age 29 from a drug OD caused by mixing too many different kinds of drugs. He regularly used something called "purple drank" which was a mixture of codeine and antihistamines (from cough syrup) and soda. The drink is also called "lean" because users would tend to lean. He also had valium and PCP in his blood.
